New Student Registration for 2021-2022 School Year
Welcome to Beautiful Summit County!
Please follow these steps to enroll a new student with Summit Middle School.
1. Gather All Necessary Documents
- Birth certificate
- Immunization Records
- New Student Enrollment form - Click here to download
- Parent Home Language Survey form - Click here to download
- Request for Cumulative Records form - Click here to download
- Proof of residency
- Prior school contact info/report cards
- Custodial papers, if needed - Bring in court documents or click here for a form that other parent/guardian can fill out and notarize
- Information on IEP or 504 needs, if applicable, bring in plans
- Middle School sports and activities need to have a sports physical on file - Click here to download a sports physical
2. Schedule a Registration Appointment
Download the forms above, fill them out and scan and email, along with your documents, to shannon.williams@summitk12.org to begin the registration process. You will then receive the information you need to complete the online registration for your student.
In August, Counselors will host a New Student/Family Orientation on August 5, 2021 from 11:30am-12:30pm at SMS. We will also schedule an individual appointment with your child's Counselor to prepare your child's schedule and to discuss any specific needs.
The first day of school is Wednesday, August 18, 2021.
